Use this checklist to ensure you don’t miss any critical steps in the return-to-work process: LINK
COMPLETE THE EH&S RISK ASSESSMENT AND PROTECTION & MONITORING PLAN FOR APPROVAL
These documents include ensuring face covering availability, physical distancing, disinfection, training, symptom screening and more. LINK
Plans must be submitted to crit@uci.edu for review and approval. No employees are allowed to return to work on-site until CRIT has provided formal approval to the Plan Owner.
INSTRUCT EMPLOYEES TO TAKE THE EH&S ‘RETURNING TO CAMPUS’ TRAINING MODULE
Send this link to all employees schedule to return to on‑site work at least one week in advance of their return date: LINK
ACTIVATE EMPLOYEES IN THE WORKING WELL™ DAILY SYMPTOM CHECK APPLICATION
Complete the attached Excel template and return it to the UCI Coronavirus Response Center no later than the Wednesday prior to any employee’s return‑to‑work date: LINK

REQUEST WELCOME BACK KITS *
Contact your local unit’s facilities/building manager for Welcome Back Kits.

The Welcome Back Kit contains the following:
Reusable bag • Branded UCI mask (reusable, washable; 1 per employee) • Branded UCI lanyard for securing mask so that it is conveniently available on the employee (1 per employee) • Non‑branded, reusable, washable masks (three per employee) • Hand sanitizer • Coaster
